I bought my test strips that I use with my glucometer from a local discount house where I have a membership. They told me they don't file claims with Medicare, so I'd like to file a claim myself. Where can I get a form?
Beneficiaries who purchase their equipment and supplies from a supplier who does not have a Medicare supplier number cannot file a claim on their own. You have apparently purchased your test strips from one of those outlets which does not have the required Medicare supplier number.
