CareFirst fined $125K State officials have fined CareFirst BlueCross BlueShield, Maryland's largest health insurer, $125,000 for improperly denying claims. The violations were found in an audit of the company that covered 2004. CareFirst has also entered into a consent decree with the Maryland Insurance Administration that calls for the company to correct the practices that led to the claims being rejected. Insurance Commissioner Ralph Tyler said CareFirst not only wrongfully denied claims, in some cases it was failing to process the denials within 30 days, which is also illegal.

Pennsylvania has ordered two juvenile residential facilities currently housing New Jersey children to stop accepting new clients, after seven youths were seriously injured by staff members, officials said.
