This final rule establishes requirements for under the Public Health Service (PHS) Act and the Patient Protection and Affordable Care Act (Affordable Care Act). The final rule defines ``'' as a type of individual health insurance coverage, and specifies that certain PHS Act requirements are inapplicable to this type of individual health insurance coverage. This final rule also amends the medical loss ratio and annual limits requirements for under the PHS Act.

Among well-documented factors that shape political news coverage are reliance on official sources, indexing of coverage to the range of opinion among officials, and privileging of "episodes" over "themes." The Downing Street Memo controversy of 2005 embodies a clash among those media agenda-setting factors and the intense desire of Internet activists to bring coverage to an issue that most political and media elites initially ignored. This case study analyzes the brief burst of mainstream coverage of the controversy. While straight news and television coverage was pegged mostly to official words and action, activists apparently had an easier time penetrating the op-ed pages of major newspapers.

This document contains final regulations regarding the summary of benefits and coverage and the uniform glossary for group health plans and health insurance coverage in the group and individual markets under the Patient Protection and Affordable Care Act. This document implements the disclosure requirements under section 2715 of the Public Health Service Act to help plans and individuals better understand their health coverage, as well as other coverage options. A guidance document published elsewhere in this issue of the Federal Register provides further guidance regarding compliance.

C onsumers are used meeting with their doctor every year for a physical and having an annual performance review with their employer, but not everyone remembers to schedule an yearly review of their homeowners insurance. Most homeowners insurance policies are renewed each year, but many policyholders only glance at the paperwork and check to see whether the premium is increasing before filing everything away.
Insurance agents recommend that homeowners take a few minutes when the policy is renewed to check their coverage and consult with an agent or their insurance company to make sure they are adequately covered. Homeowners also should let their insurance company know when they have made major changes to their home or to their personal property.

CNN - as it does for most breaking news events - created an e-mail alias so all involved in the coverage were kept abreast of coverage plans, ranging from which reporting team was covering which angle to what event was coming in on what routers. If we have time before getting on a plane, one of the first things we check is the state's Open Records Laws (or alternatively, call a CNN lawyer) to make sure we can get a records request ready. [...] while it may sound obvious, we also check to see where to deliver the request because when dealing with state and local governments-as well as various police agencies (or in the case of Tucson and Virginia Tech, university officials) - we don't want to waste valuable time on the ground trying to figure out the logistics of delivering the request.
