Doses of three chemicals injected into Jeffrey Landrigan on Oct. 26, 2010, had done the job in executing him, but there was a set of unused, filled syringes left over. Department of Corrections Director Charles Ryan ordered the execution team to dispose of those chemicals into Landrigan's corpse against the advice of the lead executioner, a physician who advised that doing so could rupture a major vein.
