Parents registering new students should take the child's original birth certificate, immunization record, two proofs of residency and custody papers, if applicable. Social Security cards are helpful, too. Items accepted as proof of residency include lease agreements, utility bills, a home sale contract, W-2 form, or a pay stub if it includes the parent's home address. Driver's licenses are not accepted as a proof of residency; however, parents should also take a photo identification card or driver's license.
